  • Insurance verification: While we do our best to verify insurance, the information given to us by the insurance company may not accurately reflect a patient’s policy and coverage. Patients are responsible for understanding their plan and coverage.
  • Phone contact: If staff are not able to reach patients by phone, medications will not be prescribed. Please ensure office calls are answered or voicemail is enable
  • Payment: Full payment is due at the time of service.
  • Returned checks: Returned checks will incur a $30 fee.
  • Cancellations: Please cancel appointments 24-hours in advance. Missed appointments without a call will incur a no-show fee ($50 for follow-ups, $100 for the first appointment). Patients who miss three appointments in a row will be dismissed from the practice.
  • Being late to appointments: Patient arriving more than 10 minutes late for scheduled appointments may have to reschedule and be charged a fee.
  • Collections: Accounts not paid after three months will be referred to a collection agency and incur a 30% service fee. Clients with accounts in arrears will be dismissed from the practice.
  • Prescription refills: Medication refills should be requested a week in advance. Refill requests are processed on Tuesdays, Wednesdays, and Thursdays. Prescription refill requests following a missed appointment will incur a $25 fee.
  • Phone calls outside of appointment: Phone consults of more than 20 minutes will incur a $50 fee.
  • Requests for paperwork: Fees for paperwork requests vary depending on the records requested.
  • Disability: We are not able to perform assessments or complete paperwork for disability claims.
  • HIPAA Notice of Policies and Practices: The Health Insurance Portability and Accountability Act and Tennessee state law require that we protect the privacy of your personal information. For more information, click here.
  • TennCare: We are not able to accept patients covered by TennCare or Medicaid.
